New Account Requests
- The form must be completed by the person listed on the water bill. This individual will become the Account Holder and will be the only person authorized to:
- Make changes to the account
- Request key fobs
- Add or remove individuals from the account
- Anyone 18 or older will need to sign their own Usage Agreement and Waiver of Liability. These documents are automatically emailed to the email provided for each individual to sign before they can be added to the account.
- Please select Woodmen Hills/Bent Grass only if you are a resident of Woodmen Hills.

Add/Remove Requests
- This form must be completed by the Account Holder (the individual whose name appears on the water bill).
- Anyone 18 years of age or older must sign their own Usage Agreement and Waiver of Liability. These documents are automatically sent to the email address provided for each individual and must be signed before they can be added to the account.
Private/Semi Private Swim Lesson Request
This form may be submitted by anyone interested in registering for Private and/or Semi-Private Swim Lessons. Once the form is submitted, a staff member will contact you to process payment. After payment is completed, the lesson request will be assigned to one of our amazing swim instructors.
The instructor will then reach out to you directly to schedule a day and time that works best for your schedule.
If you request a specific instructor, please note that we cannot guarantee that instructor will be available.
Party/Room Rental Request Form
This form may be submitted by anyone interested in booking a party or room rental with us. Once the form is submitted, a staff member will contact you within 3–5 business days to confirm that the requested date and time are available and to process payment.
Please note that your date and time are not reserved until payment is received in full. Reservations are handled on a first-come, first-served basis, and we cannot guarantee that the requested date and time will be available.
